2008 NPC Bill Pearl High Desert Classic registration (mail in form)

This Event is Central Oregon Bodybuilding Association (COBA) event benefiting the Central Oregon Boys and Girls Club of America / Event Date is: May 3rd. In consideration of accepting this entry, I, hereby intending to be legally bound for myself, my heirs, executors or administrators, waive any and all right and claim for damages I may have against the NPC, COBA, Summit High School, their agents and representatives for any and all injuries suffered in connection with the 2008 NPC Bill Pearl High Desert Classic on May 3rd 2008, and in traveling to and from said contests.  Further I agree to hold harmless and waive and release any photo rights for media (television, magazines and newspaper) to COBA, for the purpose of promoting bodybuilding through the sale of video tapes and photos and any network, cable or local television airing.
